What is the procedure for getting a duplicate Czech death certificate?
A duplicate death certificate from the Czech Republic ("Úmrtní list") can only be obtained within the country itself. More specifically, it must be requested at the local civil registry office where the death was originally registered. The Czech Republic does not have any overseas offices that can issue this document.
While officially called a duplicate, the document is identical to the original death certificate – the only difference is the current issuance date. There is no indication on the certificate itself that it is a copy.
The most important aspect is determining who has the right to request this document. Immediate family members of the deceased (children, spouses, or grandchildren) can apply directly. Third parties may also obtain the certificate, but only with a certified power of attorney from the deceased's relatives (we can assist with arranging this if needed). Many registry offices require documentation proving the applicant's relationship to the deceased. For example, children requesting their parent's death certificate should provide a certified copy of their birth certificate. An apostille for a Czech death certificate can only be obtained within the Czech Republic itself, as the country has no overseas offices to provide this service.
The apostille process for the original or the duplicate death certificate can be handled by any third party without the power of attorney from the deceased's relatives (power of attorney is only needed when obtaining a duplicate certificate). The apostille is issued by the Czech Ministry of Foreign Affairs. However, before approaching the Ministry, the death certificate must first be certified with a special verification stamp from the relevant regional or municipal administration, in addition to the civil registry office's certification. Without this step, the Ministry will not proceed with the apostille.
