Is it possible to obtain a death certificate duplicate from the Czech Republic?
Yes, it is possible. However, this must be done directly within the Czech Republic, and the document must be obtained from the Civil Registry Office in the district where the person passed away. The Czech Republic does not have any official representatives abroad who can issue such documents.
The document is officially referred to as a duplicate death certificate, although the word duplicate is not actually indicated anywhere on the document. It looks identical to the original.
The certificate can be obtained by relatives of the deceased (such as children, spouses, grandchildren) or by a third party acting under a power of attorney issued by the relatives. In many cases, proof of relationship must be attached to the power of attorney. For example, if the request is made on behalf of the deceased’s children, their relationship must be confirmed with a notarized copy of the birth certificate.
